Family Practice Physician

Department: Medical
Location: Mishawaka, IN

As a Physician, you will be responsible for coordination of proactive care for patients with chronic conditions as well as identifying wellness and/or preventative service for clinical patients. This position will work closely with the clinical staff and will report to the Site Medical Director.


  • Utilizes appropriate and effective patient care plans which includes adequate diagnostic procedures, therapies, use of consultation and referrals, and outcomes.
  • Manages patients to assure continuity of care which includes adequate scheduling and tracking of return visits, completion of treatment plans, and follow-up of chronic problems, abnormal tests, and “no show” appointments.
  • Provides whole-person care which includes appropriate
    • referrals to the social worker, nutritionist, and mental health professionals.
    • integration of biologic, social, psychological and environmental issues.
  • Maintains adequate, concise, and legible documentation of patient care.
  • Provides and documents appropriate periodic health examinations. (i.e. preschool vision exams, serum cholesterol, pap smears, etc.)
  • Educates patients about their diagnosis, treatment, prognosis, medications, preventive care, etc.
  • Utilizes appropriate infection control measures, including universal precautions.
  • Makes scheduling changes on patient schedule only through the Medical Director.
  • Provides clinical care at HealthLinc as assigned by the Medical Director.
  • Works assigned Clinic sessions – including one evening session per week.
  • Works Saturday or Sunday clinical sessions, if assigned and as reasonably agreed upon.
  • Sees patient’s work-in patients when necessary.
  • Supervises advance practice nurses in Clinic when assigned.
  • Provides consultation and prescription signatures to advance practice nurses when needed.
  • Conducts daily patient hospital rounds.
  • Oversees residents in the management of hospital patients.
  • Writes daily progress notes in hospital charts.
  • Takes phone calls about hospital patients.
  • Participate in assigned Quality Assurance/Management in Daily Work activities.
  • Regularly attends QA/TQM as required.
  • Participates in the physician performance audit as assigned.
  • Assists in the development and maintenance of HealthLinc practice guidelines (protocols) as assigned.
  • Participates in Total Quality Management (TQM) activities. 
  • Attends Clinic’s Provider Group Meetings and hospital section meeting as scheduled.
  • Attends required in-service meetings.
  • Makes presentations in the community when requested.
  • Attends annual board, staff, volunteer meetings and/or dinners.
  • Attends HL Board Meetings as requested.
  • Represents the Physician staff at the HL Board as requested.
  • Teaches students and residents as assigned.
  • Attends regional or national community health center meetings as requested.
  • Performs other duties as assigned.




  • Graduation from an approved medical school
  • Completed Residency Program
  • Board certification
  • DEA certification
  • CPR certification
  • Indiana State Medical License
  • Continuing medical education


  • At least 1-3 years of experience as a physician (not required but highly preferred)
